Step 01

Survey details

Basic submission information and the location where this family was profiled.

Maps to the database areas table.
Keep the old code only when profiling an existing family. New families use the auto-generated code below.
Format: STAFF2-LOC-YYMMDD-SEQUENCE. Example: CL-NRL-260911-0042. The final sequence is assigned by MySQL after submission, so the preview shows #### until the record is saved.

Family details

Household composition, address and contact information.

Calculated automatically from total members − elders.
Members total: ₹0
The database stores a URL. When connected to Apps Script, the image can be uploaded to Drive and its URL saved.
After entering the member counts, the next section can automatically create the required Adult and Child member cards. You can still add or remove cards manually.

All repeated adult and child questions from the Google Form are handled here through dynamic member cards.
